Medical Statement

The purpose of this medical questionnaire is to find out if you should be examined by your doctor before participating in recreational diver training, even the Discovery Programs. "Yes" to any of the medical questions require a doctor signed medical form prior to any pools classes.

Dive Season

The open water Lake season varies between May and October. Factor such as snow & cold in the fall and spring run off (no visibility) in spring make it difficult to schedule classes in June and October. Classes in these months may be reschedualed with little warning due to saftely.
